Chronic Conditions We Treat

We provide coordinated care plans and support for the following chronic conditions:

DIABETES

The most common chronic diabetes conditions are type 1 and type 2 diabetes, both of which impact how the body uses blood sugar. Excess sugar in the bloodstream can lead to very serious health concerns.
Type 1 diabetes often appears during childhood. Type 2 diabetes is much more common, and can appear at any age. It is most common in individuals over the age of 40.

Common symptoms of diabetes:

  • Excessive thirst
  • Frequent urination
  • Irritability or mood swings
  • Feeling more tired than usual
  • Blurry vision
  • Frequent infections, including gum or skin infections

If you know or suspect that you have type 1 or type 2 diabetes, we can help you develop a comprehensive plan of care to manage your symptoms.

HYPERTENSION

Hypertension, also known as high blood pressure, is a common condition affecting the arteries. High blood pressure forces the heart to work harder to pump blood.

Left untreated, high blood pressure increases the risk of numerous serious health conditions, including heart attack and stroke.

Most people with high blood pressure will experience no visible symptoms, even when their blood pressure reaches dangerously high levels – making it especially important to routinely check your blood pressure.

If you experience chronic high blood pressure, our team can help you manage the condition, alongside your healthy lifestyle habits.

HYPERLIPIDEMIA

Hyperlipidemia, or high cholesterol, occurs when one has too many fats (lipids) in their blood. High levels of cholesterol can increase the risk of heart disease and other serious health concerns.

While high cholesterol can be inherited, it is most often the result of lifestyle choices. As such, incorporating healthy habits is a good first line of defense.

Common risk factors for high cholesterol:

  • High blood pressure
  • Being overweight
  • History of smoking

Like hypertension, hyperlipidemia often carries no symptoms, only surfacing during an emergency event such as a heart attack or stroke. It can also lead to heart disease if left untreated.

If you suffer from chronic hyperlipidemia, our team can help determine the right treatment plan for you, including medication and lifestyle changes.
